Are the assessments recognized by the courts, CPS, employers, and DOL?
Awakenings has been state certified by the Department of Health and Human Services (DOH) Division of Behavior Health and Recovery (DBHR) since May of 2004. These assessments are recognized by both Oregon and Washington Courts of Jurisdiction, Department of Health agencies, Department of Licensing and Washington State Employers.
Unfortunately, those who are hoping to resolve CDL DUI issues will need to make contact with the local contracted CDL Assessment Therapist. There contact information is available upon request.
How can I get a DUI assessment?
In order to obtain a DUI assessment you must present a complete 5 year driving abstract that is obtained through the local DOL or online. (3 year driving abstracts are not acceptable) There is a small fee for the driving abstract that must be paid at the time. Although criminal histories are not required at the time of assessment it should be noted that many Courts of Jurisdiction do not accept an assessment without first providing collateral information to the assessment therapist. Releases of information to the courts can be signed at the time of assessment to insure the court will accept your assessment.
What's the cost of an assessment?
Assessment fees are $200.00 for customers paying cash. Cost of co-pay with people having private insurance is insurance specific. There is no fee for individuals with medicaid. Sliding fees are available for those who qualify.

How quickly can I be seen for an assessment?
Are assessments are generally scheduled in advance. We make every effort to complete an assessment within 7 business days after your initial contact. Due to the nature of the illness it is customary for a number of appointments to be missed. This allows us to offer stand-by appointments daily.
Can family be involved in treatment?
Since substance use disorders are a family illness, it is vital to educate and support those members who are interested in their family members treatment to be engaged in services. Although we are willing, and have a desire to educate and support families the patient ultimately determines if we are allowed to do so. If a person is over the age of 13 and does not wish to sign a release of information (ROI) our agency, by law (HIPPA & 42CFR), is prohibited from disclosing wither or not a person is involved in our services. We can however, allow people to disclose information to us about a potential patient providing we do not acknowledge that a person is a patient.
How long does treatment last?
Substance use disorders, at times, are complicated and require extensive services. The length of time of services varies on the severity of your disorder. It generally takes 6-9 months to stabilize and additional 6-10 months to develop effective recovery skills.
